MAINTENANCE OF METABOLIC INTEGRITY IN THE BRAIN AFTER TRAUMA
MRC Trauma Unit University of Manchester
MRC Toxicology Unit Carshalton, Surrey
Abstract
The metabolic integrity of the brain depends on a continuous supply of oxygen and substrate (glucose). It can be threatened by all forms of head injury and also by extracranial trauma. Although the distrubances in physiology leading to disruption of the cerebralmetabolic balance in these conditions are becoming better understood, there has been little direct investigation of the biochemical changes in head injury. However, the results of the large amount of experimental and clinical work on the loss of metabolic integrity in cerebral ischaemia/hypoxia, related to stroke and acute hypoxic states, can be applied to the injured patient. The aims of theraphy are becoming a[[arent but the methods currently used often fall short of requirements for those patients where the metabolic integrity of the brain is in doubt.
